1. Create an account on the GreenShield+ web portal*. Make sure to select the “Through an employer or organization” option and use your Member ID.2. Register for Direct Deposit (if you don’t want to receive your reimbursement as a cheque).3. Select “Submit a claim” and then “Other health specialists.”4. Enter all the details requested, and submit your claim.*Note that gender affirmation care claims can't be submitted on the GreenShield+ mobile app—they can only be submitted on the web portal.If you’re experiencing issues when submitting your claims online, please use the email or mail submission options below.
1. Download the GreenShield Health Claim Form and fill out the required information.
2. Sign and date the form and attach the required documentation, including receipts for your procedures and diagnosis of gender dysphoria.
3. Submit your claim:
a) by email to [email protected] or
b) by mail to GreenShield at this address: PO Box 1652 Windsor, ON N9A 7G5
