Important: Keep copies of your receipts in case you’re asked to send them in. Your claims can be audited anytime in the 12 months after you submit them. You’ll also need them for your taxes.
Before claiming, create a profile on Desjardins Insurance’s portal and enter your information. Once your profile is created, you’ll be able to track all your submitted claims. For more details, visit the Desjardins Insurance website.
Make sure you’re registered for direct deposit so you can have your refund deposited directly into your bank account. You must be registered for direct deposit to submit claims online or through the app.
Desjardins Insurance must receive your claim within 12 months of the date the service was incurred. E.g. If you visit a health practitioner on Sept. 30, 2024, you have until Sept. 30, 2025 to submit the claim for that visit.

1. Show your card to your pharmacist, dentist, or other health practitioner. If they accept direct billing, you won’t have to pay the full amount of your bill up front.
2. Your claim will be processed in accordance with your Plan coverage. The practitioner can tell you right away how much you have left to pay, if anything, and how much is covered by the insurer.
You can download your card on the Pay-Direct Card page, the Omni mobile app, or Desjardins Insurance’s online portal.
1. Submit your claims through the Omni app from Desjardins Insurance, available on the App Store or Google Play.
2. Log in to the app. If you haven’t registered yet, select Register and follow the on-screen instructions.
3. Once you’ve logged in, select File a claim, then follow the instructions.
Note that access to some features of Desjardins Insurance’s online services may vary by Plan or province.
1. Go to the Desjardins Insurance portal.
2. Log in to your account. If you haven’t registered yet, select Register now and follow the on-screen instructions.
3. From the home page, select Submit a claim, then Online claim.
4. Follow the on-screen instructions. If you need help, see the step-by-step instructions for submitting a claim.
If you can’t create or access your account on Desjardins Insurance’s mobile app or web portal (e.g. if you’re no longer a student), you can submit a claim form. Go to the Claim Forms section to download the form you need.
1. Fill in all the required information, including your Group Number and Certificate Number (indicated above), attach your original invoices, and sign and date the form.
2. Choose option A or B below to submit your form.
A. Submit through the secure online service: I. Go to desjardinslifeinsurance.com/send. II. Fill out all required fields in Step 1 – Information. III. Under the question What type of insurance are you contacting us about? choose Group insurance through your employer, a group or an association. IV. Choose the Request Type for the claim and enter your Group Number and Certificate Number (indicated above). V. Under Attach your documents, click on Add to upload the form and your required documents. Don’t attach zipped (compressed) files or password-protected files, and make sure your attachments are within the size limit and in one of the accepted file formats indicated on-screen under the boxed text. VI. On the next page, double-check all of the information, make corrections if needed, then click on Send. NOTE: If you click on Exit, your claim will be cancelled.
B. Submit by mail: Send the claim form and all required documents to the address on the form.
Claims submitted through the app or online are processed within 48 hours, meaning you’ll get your refund faster!
