Vision Coverage

The vision coverage is included in the Health Plan.

The current coverage period is from Sept. 1, 2024 – Aug. 31, 2025.

Please note that receipts for eyeglasses or contact lenses must include the prescribed strength or provide the detailed prescription.

Eye Exam

Payable amount of $60 per insured person per coverage period, including eye refraction.

Eyeglasses and Contact lenses

Payable amount of $150 per insured person once in any consecutive 24-month period, including replacement costs.

Replacement or Repair of Eyeglasses if Accidentally Damaged

Payable amount of $100 per insured person per coverage period.

Eyeglasses after a Cataract Surgery

Payable amount of $200 per surgery per insured person, on condition that they are required by an operation to achieve a minimum visual acuity of 20/40, at the rate of a single pair.

Laser Surgery

Payable amount of $150 per insured person per coverage period.
